Has anyone tried this? I received this in the mail a few months ago, but didn't do it. Now its on here with a $1 reward. I did it, easy enough, but still haven't gotten credit. Any insight from anyone?

If you received a survey in the mail from them a few months ago, then you already signed up with them at some point in the past.  You won't receive credit for doing the offer here as you're already a member.  The reward is only for new sign-ups. 

Our household signed up at some point years ago.  We get their surveys a few times a year.  Whenever I see this offer pop up here on FC, I just hide it from my list of offers.
