Title: Fax Coversheet for Faxing identification instead of doing phone verification

I am unable to do the phone verify so I need to know how I obtain the special cover sheet in order to fax a copy of my California I'd and I also need the fax number to fax it to.  Thanks in advance.

You should probably fill out a support ticket.

How do I go about opening a support ticket?? Is it here on the forum? If not, please let me know where on this site I need to go to open a ticket.  Thanks in advance for your help. :)

Well I finally figured out how to submit a ticket.  I submitted one for non credit of the half cent videos and now I will submit one regarding the fax cover sheet.

Half cent videos aren't credited until you have watched two for a total of one penny. Support can't help you with offers that didn't credit unfortunately, but going forward make sure you have watched two for credit.
