I posted this yesterday as a comment on another post, but it worked so well today I decided to post it again where people could find it easier.
I have a Windows 10 computer and usually use Chrome, but the problem of the survey page coming up blank was finally solved for me by switching to Firefox.
I used Firefox and when the page told me it would not load I entered Shift Control M to enter the Mobil Simulator
I did find that if I clicked where it said Relative and selected the iPad mode, then right clicked on the blank page and clicked the refresh symbol (at the top) it brought the survey page up, then I could drag it wider and it was just about normal.
I also discovered that when doing a string of DQ's I have to reselect iPad and refresh the screen again as well as reset the width between each survey. It's not as bad as it sounds, especially if you Edit the list of devices to only leave the iPad checked to cut the time down to just a few seconds between each one.
I tried it again this morning and it worked for an hour or so with no problem. Unfortunately none of the surveys would complete, but I only had time for a dozen or so and will do more later today. I was just happy to do surveys again.

Thank you for the hints and tips.
However, I am limited when it comes to my knowledge of technology.
I think that I am going to have to sit surveys out until the glitch is fixed, probably tomorrow.
Since you have figured out how to use the survey page, what is the problem? Why is this happening?

It seems to be a missing update that didn't get installed somewhere in the configuration.

The "tools" that run from the developer tools say the date format is wrong, other debugging info too. Not on the user end, it's on FC side. When they do an update is works for awhile then back to the "can't handle the request" error.

Every few days I get an error on the survey page and then the next day it will be back up. I wish they would get it fixed since we don't seem to be getting paid for the forum postings.

Let's hope someone in support will see this.
I am going to submit this post when I request my next forum bonus.

Way over my head, I take a more empirical approach like if it ain't broke don't fix it. If it don't work try later.

I hope it works for me!
