Off-Topic / HOW DO I GET THE BANNER???

NEWBIE here (or PROBIE - I watch too much NCIS....lol).....I would like to know how to make that banner I see at the bottom of some of you guys posts.  The one that shows how many offers you completed and how much you earned.  I want to post it on mine but can't figure it out, duh?

Support / Credit for Offers Completed

I am not sure why this is happening but........about every 2 out of 5 offers I complete I don't get credit for.  The offers I haven't received credit for, I have received the confirmation e-mails and confirmation the offer was completed from the individual site.  It is listed on the "All Offers Clicked" page but I haven't gotten credit.  I have a total of 5 now.  I make sure I always go through the link, I use Internet Explorer 7.0 as my browser.  Any ideas or help as to why this is happening?
