Membership verification page on surveys??!!!

In the picture below I have to verify that i'm a member of samplicio surveys. I know this for a fact because I entered in my FC info and it didn't work, I also contacted the company and got no response... If anyone ran into this when doing surveys please help

P.S. I erased my info that's why it looks funny

P.S.S. Just checked my email I did get a response(from Life fun and everything not samplicio) I did a survey from that company which was through samplicio's portal.  however the verification page was LFE's so confusing and they said I should take it up with FC but we know it's the advertiser that credits...
