It's a toss up for me between Mulholland Drive and Lost Highway. Both of them have scenes that I remember absolutely terrified me in the theater. Nothing typically "scary" in the Hollywood sense, but just scenes that were so full of dread that they really scared me. In Mulholland Drive, I think it was that Club Silencio scene, and in Lost Highway it was the videotapes that arrived at the couple's house showing them sleeping in their bedroom.
