Another Commercial Survey Glitch / Problem

I completed two Commercial Surveys today. One went well and I was credited for it but the other was a different story. I finished the second one and reached the place where you get a bar and asked to go to a number -- basically the end. I did so. I reached the number requested and entered. This is where I should have reached a screen telling me that there were no more pages and then another screen congratulating me. That did not happen. Instead I got a blank white screen and that was that. I left that page open for several minutes but never got anything more than a white screen.  Below this sentence, you will see what was in he address line.

I never did get credit for the second survey. Any suggestions about what I could have done differently or why this happened?
