people say this sometimes.
to me, it means that the person misses the other person.
it can also mean that they don't feel good and don't want to say that.
it can mean that the person has a problem that is difficult to solve.

I agree with hitch0403, sometime it's better to say I've been better than to explain every thing to another person.
