Surveys are always a hit or miss even when they come in an email invite. You can open a support ticket but there may be nothing FC can do. Be certain to take your time when taking a survey reading each question carefully. if you go too fast or your answers contradict each other, the computer will throw out your survey and there ill be no record of having taken the survey.
