I've had some survey qualifications start by asking for my first and last name initials but not name. I only consider providing that info if I've qualified for a larger study.
Opinion: 1) a view, judgment, or appraisal formed in the mind about a particular matter 2) belief stronger than impression and less strong than positive knowledge

I usually don't mind giving out my email address and name, but I don't like to give out my phone number.  When I DO have to give my phone number, like bostonphil, I give my home phone number, not my cell.  I don't recall having had any issues with giving out this info.

So many surveys where the first question is your Name, then they want more info.

I never provide personal info so I just close those surveys out and on to the next.

Has anyone actually completed surveys and provided there personal info?

You do know that even the ones that do not ask for your name DO in fact have your full name, your address, and your email address....right? Because they do.  They know who you are.

I do, but only if they are going to send me a product to test. Otherwise, nope. They can probably get (or already have) my personal info anyway, but I won't willingly hand it over without a reason.

Usually you have to provide that info to start with.  Then you find you do not qualify for the test.  I think they need to make sure you qualify for the product test before asking for mailing info.

Yes somethime they ask for your name and other ersonal information such as your address etc.

Surveys should not get too personal, in todays environment, and yes I just close them when they get too personal....

I have seen many surveys recently where3 they ask for your name too. I also close those surveys out. I have also seen some that ask for a full address, which I also do not provide. The only thing I am okay with providing is an e mail address. I would certainly never provide a social security number or credit card/banking information.

I have come to the conclusion that it depends on the survey company whether or not I feel comfortable giving name, address, phone, email etc..  These familiar companies have paid me hundreds of dollars over the years and if they ask, I give it to them.  Unfamiliar, no, I "X" out of those.  Just try and remember the ones that pay, they probably know who we are anyway from the IP address.
