Yes!  Is that the form that needs to be sent with a picture id?

Yes you need to sign that form and do a copy of your id and send it to the number on the letter

Thanks so much, I submitted a support ticket and have not heard anything.  When I tried to cash out it stated that I need to do a fax verification. I did find a fax cover sheet and a fax number I guess just have to play the waiting game.

I have sent my fax verification, I have received a response that FC received the information.  I am still waiting for a response, it has been over a two weeks.  Has anyone ran into this issue?

I emailed in my verification form on 11/13/17 and still have not heard a thing from support. Does anyone know whats going on with them? I've been trying to cash out now since 10/20/17, with no help from support.
