I too, don't mind name, address and email (I have one JUST for these situations), but I draw the line at phone #.  I like those surveys that at the 1st question ask you if you're willing to use your webcam or microphone - -I x out of those immediately

If I have time and inclination I don't mind the webcam surveys and have done several successfully.

Well since they can record you and I do not know what they will do with the video and do not feel they need my picture I just tell them I do not have a web cam.  I know I could "successfully" complete the survey.  I just choose not to.

I agree with you  countrygirl12.  I hate it when that happened  with the phone calls later. I didn't ask the to call me and I refuse to talk to them. So I have just "x'ed'  them out also. And I  do not like to  do the web cam's.  Infact I know have a piece of tape over the camera. Just in case.

I just finished a long survey on a different site. The very last question wanted my first and last name. I entered "never" and "mind". We'll see if I get credit for it. LOL!

LOL  That is hilarious.  I have done similar but never used those words before.  I am pretty sure I still got credit.  Like I have said before this is the only site that takes money back from you after it credits to your account.

THIS IS THE ONLY SITE that has ever taken money from my account after it credits.

I agree, I think they should ask for this information early ( like on the first page)not after you have completed some pages; I appreciate when it is requested upfront that way I can decline to participate if I choose to.

I totally agree, that information should be on the first page, not the last.  As far as webcam goes, no way will I do that.  I just X out if they ask.
