I did qualify for the first portion of the survey, but I ended completely forgetting about the second part. 

Update:  After all the hassle and then only earning partial credit, now the $2.50 I earned for doing this survey has been revoked!  Which means I officially earned a big fat "ZERO" for my effort.  I'm not going to bother submitting a support ticket because the problem isn't on FusionCash's end so there's nothing they can do about it.  Oh well, live and learn!

That's horrible!! I agree with IceKittyNM, try contacting FC's support anyway. Maybe they can at least tell you why the $2.50 was revoked...and why it was $2.50 and not the promised $5.00.

I actually completed one of these surveys,it did not return to the FC page, and I was never credited. Have submitted a support ticket, and still no response from FC.

I did this offer as well. I was credited for the first part. They said that they would send me a link to take the second part in the store, but I have never received the link to take the second part and it has been about two weeks now.
